Example letter for a friend or family member to collect your prescription
If you're self-isolating at home with coronavirus symptoms, a friend or family member can collect your prescription from your local pharmacy. Find out more about how our prescriptions are changing during coronavirus.
When they go, they should take:
- a handwritten or printed letter, based on the example below and signed by you
- some ID, like a passport or driving license
They only need to take the letter on their first visit during the date it covers.
But they need to take their ID every time they go.
